A new student registration form will not be accepted without a formal interview with the school principal. Upon acceptance this form, the registration fee, and the following documents are required to process your child's enrollment:

• Certified Birth Certificatate
• Social Security Card
• Baptismal Certificate
• Current Immunization Record
